Official Rules of The Thinner Winner Contest by Level Up Fitness ("Contest")

Entry and Eligibility to Win
This Contest runs from 18 June 2023 to 12 August 2023.
To enter the Contest, you must complete a registration form at any Level Up Fitness outlet in Kuching; and pay the Contest registration fee which covers the cost of services provided to you and administration of the Contest.
This Contest is open to all Kuching residents aged 16 and above by 17 June 2023. Employees and instructors of Level Up Fitness and their nominees may register for the programme but are not eligible to win the Cash Prize. Participants below the age of 16 may register for the programme but are not eligible to win the Cash Prize.
Participation in this Contest is non-assignable, non-transferable, and non-refundable without prior approval from Level Up Fitness.

Weigh-ins and Weigh-outs
You must complete weigh-in and weigh-out sessions at Level Up Fitness Premium Viva in the manner prescribed herein to be eligible for winning this Contest.
You must complete one initial Official weigh-in between 18 June 2023 until 24 June 2023, between 10:30AM and 9:30PM at Level Up Fitness Premium Viva, to provide Level Up Fitness with your weight. Official weigh-out must be done on the final day 11 August 2023.
All measurements must be taken at Level Up Fitness Premium Viva only and by prescribed Thinner Winner Contest trainers for them to be considered official.
Participants who wish to take their measurements outside of Level Up Fitness Premium Viva may do so but the results will not be considered official.

Judging and Winners
The Participant with the highest Total Weight loss points, Attendance & Physical test points.

Prizes
The Participant with the Total Weekly points and Assessment points among all participants will receive RM800 Cash Prize. Prizes are non-transferable and non-refundable. Prize Winners shall be responsible for all other fees, taxes, surcharges or any other incidental costs.
Unclaimed Prizes may not be awarded in the event of: (a) potential Winner cannot be reached for whatever reason after a reasonable effort has been exerted based on his or her information stated in the Contest Application Form, (b) noncompliance with the above or within any of the aforesaid time periods, (c) potential Winner is found to be ineligible to enter the Contest or receive the Prize, (d) potential Winner cannot or does not comply with these Official Rules.
In the case where the potential Winner is disqualified from this Contest, an alternate potential Winner shall be selected at Level Up Fitness' sole discretion, from amongst other eligible entries received.

Publicity
By entering this Contest, you consent to the use of your name, photograph(s), voice, testimonial(s), approved biographical information, or other likeness and/or information or personal exposition (and/or any edited portion thereof) for promotional, advertising, and/or publicity purposes in any Media, now or hereafter known throughout the world in perpetuity, without compensation or notice to, or further consent of, the Participant to the extent permitted by law to Level Up Fitness.

Winner Affidavit and Release
Winner will be required to provide clear copies of recently-dated Before (full-body shot) and After (full-body shots) photographs of themselves to Level Up Fitness to be used for promotional and marketing purposes.
Before receiving the Prize and as a condition of receiving the Prize, Winner may be required to provide valid photo identification featuring his or her name and address as it appeared on the Contest Application Form supplied, provide proof of proper age and residency, and sign an affidavit of eligibility and release supplied by Level Up Fitness which, among other disclaimers: (a) releases Level Up Fitness Sdn. Bhd. and its range of subsidiaries, directors, agents, agencies, affiliates, promoters, officers, employees and related persons from any and all liability related to this Contest and the receipt and use of the Prize, and (b) grants Level Up Fitness the right to publish the Winner’s name, photograph(s), voice, testimonial(s), approved biographical information, or other likeness and/or prize information or personal exposition (and/or any edited portion thereof) for promotional, advertising and/or publicity purposes in any Media, now or hereafter known throughout the world in perpetuity, without compensation or notice to, or further consent of, the Winner to the extent permitted by law.

Assumption of Risk, Waiver of Liability, and Indemnification
By entering this Contest, each Participant agrees to follow these Official Rules and to release, discharge, and hold harmless Level Up Fitness and their affiliates, subsidiaries, and agents, together with the present and future officers, agents, employees and representatives of each of them, from any and all responsibility or liability whatsoever for any property loss, damage, personal injury, or death, in connection with the Prize(s) and/or any person’s participation in this Contest.

Membership
Participants who are Members of Level Up Fitness and/or LUF Xpress are required to keep their Membership active throughout the duration of this Contest. Freezing or termination of your Membership during this Contest period is prohibited.

No Guarantee of Weight Loss
Individual weight loss results vary and participation in this Contest does not guarantee weight loss. Factors such as diet and genetic makeup, overall health, or physiological differences may influence weight loss. Please consult with your physician before beginning any new nutrition or exercise program.

General
No entrant shall use any artificial or inappropriate means of weight loss or alteration in body composition in order to win this Contest, including but not limited to liposuction, weight loss surgery, amputation or other similar procedures.
All interpretations of these Official Rules shall be made by Level Up Fitness, whose decisions and interpretations shall be final and binding. Level Up Fitness reserves the right to cancel, terminate, or suspend this Contest or any part of this Contest for any reason.
Level Up Fitness reserves the right, at its sole discretion, to disqualify any individual it reasonably suspects or believes to be tampering with the entry process, any part of the Contest itself, or the award mechanism by any means or suspected to be acting in violation of these Official Rules or any other terms and conditions of the Contest set by Level Up Fitness. If Level Up Fitness determines, in its sole discretion, that unforeseen events have compromised the integrity or viability of this Contest, Level Up Fitness reserves the right to void the entries at issue, and/or terminate the relevant portion of this Contest, including the entire Contest, and/or modify the Contest.
Level Up Fitness shall not be responsible for lost, incomplete, or late entries, weigh-ins/body composition tests, or for any entries or weigh-ins/composition tests/essays/photos that are not received by Level Up Fitness by the applicable deadline for any reason whatsoever.

Organizer
The organizer of the Thinner Winner Contest is Level Up Fitness Sdn. Bhd. (Wisma Everrise, Lot 376, Jalan Petanak, 93100, Kuching).

Declaration*

I declare the above information is truthful and correct to the best of my knowledge. I have read a copy of The Thinner Winner Contest by Level Up Fitness Official Rules, and I agree to abide by them.
